Clarify "other name" question #68

If this is a question states actually need, should clarify what the data format is, and whether this should be entered as other full names (aka after getting married), other first names, other last names, and how to list multiple, in case this helps them automate their data processing

This also looks like it will vary state-to-state, and this is placeholder. I've seen this example, and also seen in the RI example they ask for any nicknames or short names you're known by. Will keep this open to remind us to verify with each state what they're preference is.
