Form field validation #10

In general, fill in missing form field validations throughout

it's case by case -- some fields are required, others optional. the general principle that states use is that they'd rather have ppl submit an incomplete app than not complete one at all -- so we're allowing for lots of options to advance w/o filling in fields.
