Creates a unique patch file for Asterisk 20.04.0 (patch remains unchanged from 18.19.0, with #8).
Updates README.md to include both Asterisk 18 and Asterisk 20 as has been done previously.

Note: This PR includes PR #10

Please rename the patch file cisco-usecallmanager-20.4.0.patch (ie: drop the leading 0 from the minor version).

Good catch, I'm not sure how that zero ended up there.
