XDS validations being skipped #511

XDS validations for creationTime, healthcareFacilityTypeCode, languageCode, practiceSettingCode, sourcePatientId, and typeCode (all DocumentEntry) were being skipped.

Added integration test to build to cover this issue.

Added test 11998a to Registry test collection (Conformance Tool) to cover it in vendor registries.
