Define standard format for References #98

If we define a standard format for references, we can eliminate formatting issues in the current references.md file and instead format them in a template.

Marking this as wontfix for now. Changing all of the references to this format would take a good bit of time and we don't really have a compelling reason for it at the moment. If we decide in the future that we need to access individual reference parts, we can re-open this issue.
