Upgraded Issue Templates to GitHub Issue Forms #5

The task of upgrading the issue templates was assigned to me and I have updated them with reference to Appwrite's issue templates and they are working fine in my forked Repository.

I have added changes such as PHP versions, code of conduct on all templates
