Hello @yjqphp

Hope you’re doing well.

In order to make use of the form builder in UVdesk open source you need to purchase the extension from the Webkul store.

In our UVdesk helpdesk ticketing system, there is a functionality to add a contact form with the help of the form builder app, accordingly you can set that form into multiple websites and customers can create tickets from there and it will going to reflect in your helpdesk.

Clicking on the "Form Builder" option takes you to a page with multiple fields: mandatory, optional, and custom. Admins can choose the fields they need when creating a new form. Similarly, selecting the "Custom Field" app lets admins customize the fields available to customers when creating a ticket.
