feat: rework Clinical Outcomes section #7

Screenshot 2024-09-05 at 1 24 52 PM At present, the clinical outcomes tab is not helpful, don't know the difference between the diagnoses in the overview dashboard and those in the clinical outcomes. We can move the above table to the Overview section. This section is more designated for the unstructured and subjective clinical outcomes text from provider to provider.

This specific graphic comes from the Demographic Analysis Report. Clinical Outcomes contains this report, Follow Up Rate. I would like to be certain which visual to the overview dashboard. And in doing so, am I replacing the current visual that is there? (The Top 10 Diagnoses?) FollowUpRate
