3.11.0 migrations missing #534

When attempting to run Leaf, we ran into some issues. After some research, it appears that the db changes from 3.10.1__3.11.0.sql are not included in the main db scripts (missing [app.].[sp_GetCohortById procedure], and [app].[sp_GetServerStateAndNotifications] is missing the final version query.

Apologies as I would have created a PR request, but I'm not totally sure how the files are organized.
