Patch notification messages don't identify the patch #355

When looking back at my e-mail or chat history, it's hard to figure out which 
messages correspond to which patch sets because the notification message is 
just a plain link (http://codereview.appspot.com/123) with no indication of 
which patch set it's for.

Could the message please be extended to contain the patch set number (e.g. 
"Patch Set 4") and, ideally, the descriptive message for the patch set?
