Improve Supply logging pages - UI #373

Also: "Cash Receipt" on first page should be "Sale". (At least for now). For the Sale page, the context should be clarified as who makes the sale. "Value" field not clear for a shipment on a sale. Limit virtual accounts on a sale to those associated with the context.

Changed text on exchange in-out top box. CR -> Sale Value field clarified.

Added help to the exchange slots. (We can use this type of button for all kinds of specific help wherever needed.)

I notice there is some formatting cleanup needed on the totals.
