Firefox bug on selected, need to name the form #415

Just fixed a problem in process logging where in Firefox (only) the selected item was not showing up. The field was using chzn. Adding a name= to the form fixed it. (Known Firefox bug.)

Since sometimes we test only in Chrome, we should make a pass over all our forms and add a name.
