ENH: Big refactor. #95

Lots of this PR is incorporating the new self._retrieve_dt and self._retrieve_vintage methods for each relevant scraper
Changes the Arkansas scraper to merge using fips codes rather than county names
Changes the base class to restrict the insert with county names to only fips>100 (to prevent the Arkansas bug from biting us in the ass again)
Fixes the Michigan scraper to use cumulative tests rather than daily
Adds Florida to the corresponding init locations

@sglyon -- I don't think you'll be able to review this today, but I'm going to go ahead and merge because I think there are some important fixes in here.

Please take a look at this post-merge. We can always revert and fix these things.
