Properly handle 'mount_url' in "Insert Image" and other locations #114

RapidApp has recently had support added for mounting at a different sub URL via Plack (exposed via $c->mount_url) however this has not yet been handling in all edge cases, such as in the HtmlEditor "Insert Image" which still calls paths hard-coded at /simplecas/..
