Add support for multiple ldap servers, each with own forest (not a failover). #250

What steps will reproduce the problem?
If you support multiple domains, that cannot be merged under one forest, you 
have to run multiple copies of PWM.

What is the expected output?
During initial configuration allow for adding multiple ldap servers (not for 
fail-over), each with own connection parameters.
User can choose different domain (server) for all tasks.

What version of PWM are you using?
Tried 1.6.1, 1.6.2. Probably not going to use, if there is no multiple forest 
(servers) support.

What ldap directory and version are you using?
Active Directory (servers 2003-2008r2, 2008r2 schema)

Please paste any error log messages below:
None, feature request.

This is beyond the planned capabilities of PWM.  Recommend you look at an ldap 
proxy solution or some other identity management approach to consolidate your 
identity environment.
