Signature form correct for non-std SIGHASH values #123

Fixes edge case where SIGHASH_ALL = 0 instead of 1

Newer BIPs leave open the door for non-standard sighash values

Malformed Txs with sighash = 0 (sighash_all) exist, such as this test vector:
