Implement username bans. #7

Implement command to allow Discord server admins to ban the username of a user to prevent them joining back with an alt account.

This should also be able to link up with a discord server so it can see if that server has banned a username and keep that username banned from the server. - This is a slightly more advanced use case and I'm happy to raise as a seperate issue
