Drop support for EOL Rails and Ruby version #145

This PR drops support for:

Rails 4.2
Rails 5.0
Rails 5.1
Ruby 2.6
Bundler 1.11

These are all because of testing infrastructure no longer being able to run to completion and I don't want to spend time on getting the CI to work again.

The versions above might work if we're lucky but they aren't tested against, so no support for those versions is provided.
