The Order ID doesnot show up in the Attachments > Manage Attachments

What steps will reproduce the problem?
1. Create a Custom Option with AJAX file
2. Place and Order and upload the file.
3. The File shows up in the Attachments > Manage Attachments but the associated 
Order ID is not captured. It shows as N/A

What is the expected output? What do you see instead?
The Order Id associated with the Attachment should be shown

What version of Magento/extension are you using?
Magento 1.5.1.0 / Dynamic File Attachments 1.1.0b

Please, check conflicts with other extensions.
You have quite a few installed.
It works for us on a clean install.
