Add bug report template #154

I think we reached a point where too many bug issues need to be triaged. We should add a template that could let reporter list out steps to reproduce and their system info. If possible, a checklist to see if the issues already exist in Verso or Servo.

Hi, I'd like to work on this! Just to clarify, there would be a sort of pre-submission checklist section to see if the issue already exists in this repo?

@ChinoUkaegbu Let's just add steps to reproduce and system info then. I can add more requirement later on.
