Automatically overwrite titles for common websites #402

Several submitters still input a title for common links, even though we maintain a list of URLs and names for these sites.

These user written titles for common links should be overwritten (blanked out) and locked from further editing.

Add a disambiguation field for websites with multiple versions like mora, which has separate links for AAC and FLAC.
