Duplicate edit comment on adding new media #41

There is a duplicate submit comment after adding new media when the media history isn't empty. It also isn't clear what the submit comment is referring to.

When the media history is empty, the comment isn't duplicated but it still isn't clear what the comment is for.

Example (on testing instance): /db/albums-history.php?id=101230&tab=media /db/albums-history.php?id=104514&tab=media
