Revise tube map install instructions to start from zero

Right now the tube map install instructions:

• Don't cover getting (the right) Node
• Can't choose between npm and yarn
• Don't have you use the package lock and so are brittle
• Are hard to find

We should fix these so Lancet can just link to easy install instructions
