[Enhancement] Allow User to select either token OR challenge-response #542

It would be great if pwm could be set to allow the user to choose between 
multiple configured validation methods if appropriate profile settings are set 
(ie. check for presence of SMS number/email/challenge response variables and 
allow the user to select which method they would like to use).

Example use case: School setting where some students have access to SMS tokens 
which is quicker/more convenient/(possibly more secure?) but only some users 
have mobile devices and the rest could use either a personal email or challenge 
questions to reset their passwords.
