VTK reader doesn't handle multiple instances of same element in different segments #39

If an element occurs in more than one segment it will be dublicated in ViennaGrid Mesh. This problem arises mainly in multi-segment meshes where the geometric dimension is greater than the topologic dimension. e.. lines on a plane
