I don't see all your installed extensions. Can you make a screenshot of the rest of the list ?
Check with all of your extensions (keep User Themes
activated) deactivated.
Edit: You also have an older Gnome version than #914.
Can we get screenshots and a lot more infos ? (check other issues for examples)

Have you already followed the "Fix for libadwaita" instructions in the ReadMe on the main page? If that's not working I'm not sure there's a fix...newer versions of Gnome are ignoring themes intentionally on some built in apps.

I followed the instruction but the theme does not persist after reboot. What could be the problem?

I wrote this issue because the issues list was very long to search. But after I submitted the issue I was able to fix the problem by using a comment by the developer in this issue
Removing or Disabling the Custom Accent Colors extension solved my problem.
But in some apps like Extenstion Manager the WhiteSur theme is not applied. Why would that be? I think it is because its based on GTK-3. How can i apply this theme to GTK-3 apps

AFTER DOING TESTING THE WHITESUR GTK THEME WORKS AND RUNS STATELY ON THE LTS VERSION OF UBUNTU (I USE JAMMY JELFISH), AND FOR THE NON LTS VERSION OF UBUNTU, MANY BUGS ARE STILL FOUND.

SO I RECOMMEND TO INSTALL THIS BEAUTIFUL whitesur-gtk-theme
IN THE LTS VERSION OF UBUNTU, EVERYTHING SUCCESSFUL AND THERE WAS ALMOST NO BUGS
