Add more metadata for various tests beyond B12 and D3 in tests table #2

Currently the tests table only has data on B12 and D3. I'd like to add data on lipid profile, TSH, and the gastro tests I did in 2016.

I have been making more progress on this (see https://github.com/vipulnaik/diet-exercise-health/commits/master/sql/tests.sql) and will likely do more as I get closer to planning and executing on my next round of blood tests.
