Issue: Form does not reset on page reload.

Steps to reproduce:

  1. Go to https://ve-iitg.vlabs.ac.in/Experiments/commerror1.html
  2. Enter some answers.
  3. Click on 'Grade the Quiz'.
  4. Reload the page.

Expected: The form will be reset with no answers selected and no correct answers displayed.

Actual: Previously Selected answers are still selected and correct answers still displayed
