Provide support for Packrat #123

Introduces support for Packrat.

Includes doing away with the init.R file and instead detection and installation is based off the packrat/init.R file.

Fixes #110

Yes, I've refactored the buildpack significantly. Packrat and renv are both supported. Everything under the master branch now. Bit of a clean up!

The new readme is really good as well. Example apps, caching info, an easy way to set CRAN mirror... a big fan of this update :)
