The docs are organised into a main readme that has links to individual detail docs in their own directory.
The readme includes a reference to an SVG containing the latest auto-build status.
Attempting to build without write access to the install directory now generates a nice error message.
Also fixes #31 - duplicate macros (thanks Oleg).
Set version to 1.95. (2.0 is getting very close.)
