Hi described in your title is a feature, it's the Grip Holster mode, where letting go of grip is supposed to give you hand access.
Regarding the rest, if your on a controller type that hasn't had a community shared binding for HL2VRU (which I recommend you try first if existing, before DIY'ing) then all you need to do:

It may be due to my SteamVR bindings not being correctly or completely set up, as it was not clear to me from the GitHub directions how to set them up. I went into my SteamVR bindings and made sure the Dual Wielding custom binding was activated, but beyond that, I could not figure out how to bind on both controllers in the SteamVR bindings menu: "Fire", "Alt Fire", "Eject Magazine", "Open Weapon Selection" Can someone help with some clearer step-by-step instructions on how to edit my SteamVR Binding setting correctly?
