Hi,

I really appreciate your work, it is amazing! I have one question, however. I want to design a shiny app that will serve as a survey for my thesis. Based on our tests this HTML template, it's logic to be precise ideally fits that purpose. Therefore I would like to ask you, on what license is this work distributed?

Thanks for the compliment! I think Open CC is fine. Not too familiar with licenses myself, but feel free to credit me wherever it's used with some reference!
