Here we go - this is great for design inspiration (from #502):

There is a nice animation on hover - moves slightly to the left and adds shadow:

There are faces for ranking the site. Text appears on hover for each.

After clicking a rating, a text entry window appears.

It asks for your email address after you submit feedback.

Last phase: displays a confirmation that your feedback was sent.
