Backport Q1 2019 Release Fixes

Description

Backports the fixes made to the q1-2019 branch (see the release PR, #414). This includes the following release fix PRs:

• 428

• 427

• 426

• 424

• 423

• 420

This resolves #417, resolves #422, and resolves #426. These are all already fixed on master per the related PRs.
