Defect Description: In the Add Document page of Reference Documents, the upload button is not working even if we enter the type of the file and upload a valid file instead the upload button should worh inorder to upload the reports

Actual Result: In the Add Document page of Reference Documents, the upload button is not working even if we enter the type of the file and upload a valid file

Environment : OS: Windows 7, Ubuntu-16.04,Centos-6 Browsers: Firefox-42.0,Chrome-47.0,chromium-45.0 Bandwidth : 100Mbps Hardware Configuration:8GBRAM , Processor:i5
