remove deprecated stuff from v0.1.0 #38

as we go from 0.1.0 to 0.2.0 we should allow ourselves to actually remove constructs that were still around, but marked as "deprecated" at the time of 0.1.0

this includes:

the ttl_fmt as a function (see #33)
the need for the unparse function (predating xmlasdict's native suport for mixed-content - see #30)
