Changes

Remove the custom tf2 SDK pull source.
Add support for L4D and L4D2 via a custom hook format and define guard.
- I duplicated a bit of code - if you would like that only the extra parameters are define-guarded let me know.

I had seen some L4D related posts in the forum thread, but wasn't able to figure out what work was done since the https://github.com/L4D-Community/Collisionhook fork had vanished, so this is appreciated!
