clarify README instructions #22

I had to give similar instructions to a few people who wanted to run this, so I added the most common hangups. Unless I am mistaken, env.example does not exist. I have the bot running on OS X and Ubuntu and neither needed this step.

Seems like .env.example was removed by misstake in 4127e6d3fd6337e7cb79fbce2830e8c47c61d046. If you could add it together with this commit and readd the instructions for .env that would be awesome!
