Allow sending a note to the moderator(s) on the list overview page when users try to subscribe to a list #16

when a user gets to the list overview page (when not logged in), they see a page
where they can enter their name/email to subscribe to the list.

we should add another field there when the list is moderated so that the user
can include a note to the moderator that gets included with their subscription
request.

this note gets saved to the users subscription hold (as their account may not
exist yet) and if they are added to the list, then it gets saved as a note on
their account.
