This PR supersedes https://github.com/voxpupuli/puppet-squid/pull/9 and (most of) https://github.com/voxpupuli/puppet-squid/pull/18

@fluser @jfrost - This combines your PRs. Let me know if you think I've missed anything. Hopefully, it works for both of you.
