Improve output of data-driven tests in Visual Studio #509

Gallio only reports the first test instance to Visual Studio so row-tests
only appear as a single result, for example.  We should leverage Visual
Studio's concept of an aggregate test result (which has inner results) to
represent data-driven tests.
