Updates to support get slice

[i-norden]

• times_validated field in header
• resync can reset times_validated (so backfill can catch where resync hits EOF errors)
• backfill based on validation level
• index log contract address
• index both receipt contract address and the hash thereof
• fix dockerfile entrypoint script
