Commonwealth Health Physician Network-Cardiology aka Great Valley Cardiology (GVC), HQ: Scranton Pennsylvania, Emp: 53, NAICS: 62621. Breach occurred Feb 02, 2023, discovered Apr 13, 2023, notification June 12, 2023. Access was gained through a brute force attack on the “GoAnywhere” secure file transfer server affecting records of 184,764 patients and others. Information exposed: names, address, date of birth, social security number, driver’s license, passport numbers, credit/debit card numbers, bank account, health insurance, claims and medical information. Medical information includes dates of service, diagnoses, medications, and lab results. GVC learned of the hack through the U.S. Department of Homeland Security. Offered free access to Experian IdentityWorks SM for 24 months to provide ID restoration and credit monitoring services.
