Remove word "continuation'; replace with "implicit"/"explicit" #18

Now that (1) continuation docs are no longer partial DID docs, and (2) there are no implicit capabilities if the OP_RETURN field is present, I think the word "continuation" makes less sense. In v0.1, we can use the words:

"implicit": no OP_RETURN, DID document is generated), vs
"explicit": OP_RETURN present, use referenced DID document exclusively

But even in the case of an "explicit" DID Document that would be used exclusively, we're still saying that this would NOT contain the @id, still the DID is only known after it is registered.. Right?
