Determine who can submit use cases #5

We have both github issues and the google form to enable submission of use cases. We want to double-check whether submitters need to be CCG members, or whether there can be lighter requirements if we submit on their behalf.

Asked chairs and it was suggested that this is fine but that a checkbox should be added to require that contributions be by members who have signed the full IPR agreement.
