.php references should be eliminated #402

Although already addressed with #63 I can still see quite a few .php references in the published document at https://fedidcg.github.io/FedCM/ The specification should not make any assumptions on the used programming language at IdP side.

Ah thanks! I think we can just remove the extensions, I'll send a PR.
