I've started looking through this via the diff. It looks like the Editor's Note has been placed inline, but I expected it to be a separate block (which will receive the "Editor's Note" text when the final HTML is generated).

I've been trying to use respec to generate view the final HTML but I can't get it to generate HTML (I've had what looks like slightly more success when using the --localhost option, but it still won't actually generate any HTML). Does anyone know an incantation that works?

@matatk - this is how the NOTE and Editor's Note content is integrated into the document - I had to go back and double-check that as well, and authored as expected (I essentially replaced the old NOTE with the new NOTE text).
