Erratum: Mismatch on default for `commentPrefix` #801

In the metadata document it says #; in the model document it says null. I think it should be null.

My implementation uses "#" as the default value, but there are no tests to verify that it is used this way; probably because it only comes in when we're skipping header rows. I'm fine with making such a change.

If we agree on changing the metadata document in this respect, than we should remove the label on the model before acknowledging the erratum. Otherwise I am fine with it.

Summary: The decision on the call, 2016-02-03 is that the metadata document should say null
