Factoring out the 'OPTIONAL' for the parameter table. #675

All entries in the parameter table in §3.2.1 have a repeating "Support for this parameter is OPTIONAL." remark. For reasons of readability the remark has been put in the text above the table and these remarks have been removed from the items themselves.
