Initialize with content from first draft #4

I wasn't sure about authors/editors list, and there were a couple of internal refs in the original doc that didn't exist, so I left those deliberately broken for respec to pick them up as errors so we remember to fix them.

I wonder why the reference generates the URL https://w3.org/TR/did-core for did core instead of https://www.w3.org/TR/did-core. It works, but that is not the official URL for the document. If this is generated by respec automatically, then there may be a problem somewhere (possibly in the specref list) and this should be tracked down.

@iherman that's my bad, I mistyped it in the local biblio, now fixed

@jandrieu and @rhiaro : I raised a PR against this PR (see #5). If that PR is merged, then the changes Joe requested in the comment stream above will be included in this PR (as a separate commit, I believe) when it is merged.
